If you are a new patient to our practice, or if you have not been seen by Dr. Nancy R. Griner, Dr. Eric N. Plotnick, or Liza Tanghal, PA-C since October 2001, please download all appropriate forms and complete prior to your appointment. All patients should complete a patient registration form and medical history form in addition to signing a patient consent form. Filling out these forms and signing the consent form prior to your arrival will shorten your waiting time.
